REDCap User Account Request Form

Purpose

This form is for Oklahoma State University - Stillwater faculty members to request a REDCap user account for an individual who requires access to REDCap for research, academic, or administrative purposes.

Important: Requests must be submitted by an OSU faculty member. Requests submitted directly by students will not be processed.

OSU-Stillwater faculty members will need to renew their account once a year. OSU-Stillwater students accounts need to be renewed each semester.

☐ I agree to ensure that the requested user follows all applicable university policies regarding research data, privacy, and information security.
